On our first session, an initial assessment form is completed where basic information is recorded. Throughout our sessions I keep written notes, which are stored in a locked cabinet at my home address. These notes are discussed with my clinical supervisor only and are used as a valid and accurate record of your journey.

Six months following the end of our sessions these records will be shredded.
